Form 32.204 Agreement for Shared Parental Responsibility



1. Robert T. Johnson ("Father") and Martha A. Stewart ("Mother") agree to share parental responsibility of Deanna Durbin, whose date of birth is March 3, 3333 .

2. Martha A. Stewart shall be the primary residential custodian of Deanna Durbin. Deanna Durbin will reside with Martha A. Stewart at all times with the exception of visitation periods with Robert T. Johnson.

3. Commencing March 3, 3333, Martha A. Stewart shall have visitation with Deanna Durbin every other weekend from Fridays at 6:00 PM through Sundays at 6:00 PM.

4. Martha A. Stewart shall deliver Deanna Durbin to Robert T. Johnson's residence on Fridays at 6:00 PM.Robert T. Johnson shall deliver Deanna Durbin to Martha A. Stewart's residence on Sundays at 6:00 PM. Each parent shall bear their own expense in transporting Deanna Durbin.

5. During the periods when Robert T. Johnson does not have custody of Deanna Durbin,Robert T. Johnson shall have reasonable telephone access to Deanna Durbin.

6.Robert T. Johnson shall have custody of Deanna Durbin on the following holidays:

(a) Holiday from Wednesday night before Thanksgiving at 6 PM until 6:15 PM on the following sunday in 1999 and alternating every Even year thereafter. Martha A. Stewart shall have the same schedule in alternating years.

(b) Every Father's Day every year.

7.Robert T. Johnson shall have custody of Deanna Durbin on the following vacation periods:

(a) three months every summer. Martha A. Stewart shall give Martha A. Stewart 60 days notice of her intent to exercise visitation during this time.

8. Neither party shall be allowed to remove Deanna Durbin from the state without notice to the other party. Robert T. Johnson may not remove Deanna Durbin from the state during periods other than those in which he has custody of Deanna Durbin unless Martha A. Stewart consents. Martha A. Stewart may not remove Deanna Durbin from the state during periods that would interfere with Robert T. Johnson's periods of custody with Deanna Durbin.

9. Martha A. Stewart and Robert T. Johnson agree to consult one another with regard to all decisions that pertain to Deanna Durbin's care and training, and agree to have equal authority over Deanna Durbin's education, health care, religious training, vacations and travel, activities and hobbies.

10. Martha A. Stewart and Robert T. Johnson agree that each shall continue as the natural guardian of Deanna Durbin and that they each have joint and equal responsibility for the following: authority to settle any claim or cause of action that accrues to Deanna Durbin for damages to the child(ren)'s person(s) or property, to collect, manage, and dispose of the proceeds of any settlement or of a life insurance policy, and of any other real or personal property distributed from an estate or trust, to or for the benefit of Deanna Durbin during minority. The parties further agree that these actions may be undertaken without appointment, authority, or bond if the amount involved in any instance does not exceed $5,000.
